Making your appointment

Please have your insurance information and referring doctor's information ready when making an appointment with us. This gives our staff adequate time to obtain any referrals necessary and to verify the details of your insurance plan.

We are able to accommodate most urgent appointments that are referred by a physician.  Appointments are usually available within a 1-2 week period. Our earliest appointment is at 8:30 AM on selected days and evening appointments are available.

If you need to cancel or reschedule an appointment, please kindly inform us at least 24 hours prior to your office visit so we can give that slot to a patient who may need to see us urgently. If you are scheduling a new patient appointment with an EMG (electromyography), please be advised that there is a $100 fee if the appointment is not canceled with at least 24 hours notice. Additionally, there is a $100 fee for all other EMG and VNG testing not canceled with at least 24 hours notice.

Referral

It is your responsibility to assure that all necessary referrals have been requested from your primary care provider in advance. Without appropriate referrals that may be required by your insurance company, you will be responsible for any fees related to your care during that visit.
